Texas Health & Safety Code § 181.103 & § 181.105

As a patient of Apex DOT Physicals, you have specific protected privacy rights regarding your Protected Health Information (PHI) under both federal HIPAA regulations and Texas state law.


1. Your Right to Request Records 

You have the right to access, inspect, and obtain copies of your medical and healthcare records maintained by this office. To request your records:
